At work it shows up in small places: rewording an “inconclusive” label until a support lead knows what to do next, or marking which words in a manuscript must never change.
All three cases on this site come from small teams of 1–10 people; company names are anonymized under contract terms. In each one I tested with real users, ran an offline evaluation before choosing a model behavior, and agreed a rollback line with engineering before shipping a contested decision.
